The Care Coordinator will handle case management activities, assessments, patient utilization reviews, scheduling and directing case management reviews, etc. Will communicate with hospitals & physicians, etc. Will explain benefits and care options to clients and patients. Master's Degree in Psychology, Sociology, Counseling or related field. Someone licensed. Either LPC (Licensed Professional Counselor) or LSW (Licensed Social Worker).
